Important - Do not submit any worksheets that the Office of Financial Aid has not requested. Check your additional information request list.
Return your completed worksheets to Financial Aid in one of three ways:

- fax to (815) 455-8594
- leave in the drop box outside Building A, Atrium Entrance.
Do not upload V4 - Verification, and V5 - In/Dependent Verification worksheets. Email finaid@mchenry.edu to schedule an appointment to discuss these worksheets.
